1.Administrative documents for imported cosmetics
2.Taxpayer Identification Number (Taxpayer Identification Number)
3.Valid importer identification number (API)
4.A valid power of attorney specifying the country of origin and its validity period.
5.Issued by a competent government official or agency in the country of origin and certified by the Indonesian Embassy./The certificate or declaration authenticated by the Consulate General indicates that the cosmetic factory in the country of origin has implemented Good Manufacturing Practices (GMP) for cosmetics according to the dosage form to be notified.GMP(For factories located outside ASEAN countries, certification and legalization by the Indonesian embassy or consulate are required.)
